Office Closure for November 2019

 

Our office will be closed on the following dates: ​

• December 20, 2019 through Jan 6 2020 in Observance of the Holiday & New Year

• All requests for Lab Slips, Test Kits, Prescription Refills, Product Orders must be submitted no later than Friday 12/19/19.

• The after hours number for urgent triage care is 415.481.0454.Please read below for Triage Line Information/Instructions/Policies For Reaching the Doctor While the Office is Closed should an Urgent Need arise.
